Athlete Liability Waiver and Release

Agree that prior to participating, I will inspect the facilities and equipment. If I believe anything to be

unsafe, I will alert my coach or trainer of such conditions.

Facility equipment:

Use of equipment in this facility may include but not limited to: Curve self-propelled treadmill, Assault bike,

rowing machine, squat-rack, with weights and cable training system, kettle bells, dumb bells, resistance bands,

weighted balls, agility ladders, batting cage and various baseball/softball training equipment.

Acknowledgements:

I Acknowledge and fully understand I am voluntarily participating in activities on the premises that may

involve known and unknown injury, accidents, or illness; including catastrophic injury, permanent

disability and/or death, which might result from not only my own actions, inactions or negligence, but

the actions, inactions and negligence of others, conditions of the property or equipment used.

I assume all the foregoing risks as a condition of participation and accept personal responsibility for

any damages following such injury.

Release of liability:

I unconditionally release, waive, indemnify, and hold harmless and consent not to sue The Shop-

Multi-Sport Training Facility for any and all liability to the undersigned. This is for any claims for losses on account of injury, accidents, or illness, including death, or damage to or loss of personal property

while participating in any activity at The Shop- Multi-Sport Training Facility.

This waiver shall be governed by and construed under the laws of Pueblo County in Pueblo, Colorado.
